Professional Liability Insurance renewal: (For Massage Therapists only) An email will be sent out 21 days prior to the expiration date we have on file, to allow Providers to submit their new document with a new expiration date. To submit liability insurance, please include a copy of the certificate of insurance (COI) otherwise it will not be accepted.
Example: Insurance cards will not be accepted.
